I certify I am a Citizen of the United States of America of good repute. I am not a member of any organization having its purpose to overthrow the Government of the United States, or any of its political subdivisions, by force and violence. I have never been convicted of a crime of violence and, if admitted to this club, I will obey the by‐laws and rules of this club and will fulfill the obligations of good sportsmanship.

I agree that I will be accepted into the club as a probationary member for a maximum of 2 years. During the 2 year probationary period, I shall be responsible to meet certain requirements to achieve full member status:

(A) I shall attend 4 total meetings during the 2 year probation

(B) I shall participate in 2 club workdays during the 2 year probation

(C) I understand I may not have keys to any building, storage shed, etc. unless I am on a club committee and the Committee Chair presents my request to the Executive Board for approval.

(D) It shall be my responsibility to have my card marked for all required activities.
